Conclusiones
Se presenta el caso de una paciente con Cefalea Vascular y Crisis Comiciales, sin manifestaciones hemorrágicas secundarias a una Malformación arteriovenosa grado V según Spetzler-Martin.

Este sistema se basa en tres componentes:
Tamaño de la malformación
Pequeñas (< 3 cm).................... 1 pto
Medianas (3-6 cm) ..................... 2 ptos
Grandes (>6 cm) 3 ptos
Elocuencia del cerebro adyacente
No elocuente ............................. 0 pto
Elocuente................................... 1 pto
Patrón de drenaje venoso
Solo superficial.......................... 0 pto
Profundo................................... 1 pto
Grado= (Tamaño) + (Elocuencia) + (Drenaje venoso)
Elocuentes: Cortezas motora y sensitiva, tálamo, hipotálamo, cápsula interna, tallo cerebral, pedúnculos cerebrales y núcleos cerebelosos profundos
Las malformaciones arteriovenosas grado I son pequeñas, superficiales y ubicadas en zona no elocuente.
Las lesiones grado V son extensas, profundas y situadas en áreas neurológicamente críticas.
Las malformaciones arteriovenosas mayores de 6 cm. se clasifican en gigantes y por tanto serán siempre de 3 o más puntos. (De forma más frecuente 4 y 5).
En las lesiones grado III, últimamente se ha propuesto una modificación por Oliveira considerando que éstas pueden ser divididas en lesiones grado IIIa y IIIb, siendo las primeras las ubicadas a nivel de lóbulos de los hemisferios cerebrales, y las IIIb las situadas en núcleos de sistema estriado, tálamo o tronco cerebral, y que a pesar de tener ambas el mismo puntaje las últimas conllevan un riesgo diferente desde el punto de vista quirúrgico.
