Programa de prevención previo a tratamiento biológico
Paciente de 38 años diagnosticada con ESPONDILITIS AXIAL tras meses tomando analgésicos (anti inflamatorios no esteroideos y corticoides) y sin mejoría.
DIAGNOSTICO: Espondilitis axial

RESUMEN:
Paciente de 38 años diagnosticada con ESPONDILITIS AXIAL tras meses tomando analgésicos (anti inflamatorios no esteroideos y corticoides) y sin mejoría. Se le propone comenzar tratamiento biológico, ya que en la actualidad han sido un gran avance, al haber conseguido minimizar la inflamación que poco a poco va dañando las articulaciones en la ESPONDILITIS AXIAL, ANQUILOSANTE Y ESPONDIOARTRITIS de este tipo de pacientes. Generalmente lo que se consigue con estos tratamientos es modificar, de alguna manera, el desarrollo de la enfermedad inhibiendo la inflamación, y aunque no quiere decir que la enfermedad se detenga, si evita, en gran parte, que las articulaciones se bloqueen o dañen, que es uno de los grandes riesgos de dicha enfermedad. Los tratamientos biológicos pueden ser inhibidores del TNF (anti-TNF) y los no anti-TNF. Es muy importante cumplir meticulosamente este tipo de tratamiento, nunca suspenderlo sin una orden médica. En éste caso el tratamiento consistía en una inyección subcutánea diaria, la cual, era el propio paciente el que se la pondría a sí mismo en su domicilio.
INTRODUCCION:
Una vez que se toma la decisión, tanto por parte del especialista como por el propio paciente, de comenzar con un tratamiento o terapia biológica es importante que dicho paciente conozca los signos y síntomas que éstos tratamientos podrían provocar y llegado el caso, saber también cómo actuar independientemente de la patología que sea.
También es importante no olvidar las medidas higiénico-dietéticas, puesto que ayudarían a disminuir algunos de los riesgos. Por lo tanto es vital vacunarse y descartar cualquier enfermedad posible latente como por ejemplo:
- Infecciones por virus de la Hepatitis B o C
- Infección por virus de inmunodeficiencia humana (VIH)
- Tuberculosis
- Etc…
OBJETIVO:
Nuestro objetivo es conocer lo importante que es antes de iniciar un tratamiento biológico, SABER COMO ESTÁ EL PACIENTE INMUNIZADO frente a las infecciones a las que éste se expone y por lo tanto, seguir el programa de vacunación pautado estrictamente.
PROGRAM OF PREVENTION PREVIOUS TO BIOLOGICAL TREATMENT
DIAGNOSIS: Axial spondylitis
SUMMARY:
A 38-year-old patient diagnosed with AXIAL SPONDILITIS after months taking painkillers (non-steroidal anti-inflammatory drugs and corticosteroids) and without improvement. It is proposed to begin the biological treatment, since at present they have been a great advance, having been able to avoid the inflammation that is gradually damaging the joints in the AXIAL SPONYLITIS, ANQUILOSANTE and ESPONDIOARTRITIS of this type of patients. Practically what is achieved with these treatments is to modify, in some way, the development of the disease by inhibiting inflammation, and although it does not mean that the disease stops, if it prevents, in large part, that the joints are blocked or damaged, which is one of the great risks of said disease. Biological treatments can be inhibitors of TNF (anti-TNF) and non-anti-TNF. It is very important to meticulously comply with this type of treatment, never suspend it without a medical order. In this case, the treatment consists of a daily subcutaneous injection, which, it was the patient himself, who would put himself at home.
INTRODUCTION:
Once the decision is made, both by the specialist and by the patient himself, to start with a biological treatment or therapy it is important that said patient knows the signs and symptoms that these treatments could cause and if necessary, also know how act independently of the pathology that is.
It is also important not to forget the hygienic-dietary measures, since they would help reduce some of the risks. Therefore it is vital to get vaccinated and rule out any possible latent disease such as:
– Hepatitis B or C virus infections
– Human immunodeficiency virus (HIV) infection
– Tuberculosis
– Etc…
OBJECTIVE:
Our goal is to know how important it is before starting a biological treatment, KNOW HOW THE IMMUNIZED PATIENT IS AGAINST the infections to which he is exposed and therefore, follow the vaccination schedule strictly prescribed.
DESARROLLO DE CASO PRÁCTICO:
PROGRAMA DE VACUNACION
Paciente derivado de: Reumatología
Diagnósticos: espondilitis axial desde hace 1 año, aprox. Ha sido estudiada por parte de Neurología donde se han descartado Infección Tuberculosa Latente
Alergias conocidas: medicamentosas desconocidas. No alergia específica a la proteína del huevo, neomicida o gelatina.
Tratamiento habitual: ahora mismo sin tratamiento (último tratamiento eterocoxib). Pendiente de iniciar tratamiento biológico
Antecedentes vacunación: refiere vacunación en la infancia, pero no dispone de documentación. Recuerda haber pasado varicela en la infancia, no sarampión. Refiere vacunación antitetánica, pendiente de traer cartilla amarilla. No refiere vacunación antigripal en campaña 2015/16 ni anteriores.
Transfusiones: no
Serología:
- 14/05/2016: IgG sarampión (+);IgG varicela (+);AntiHBs (-) 2.0 Ul/mL; antiHBcActot (-); HBsAg (-); AcVHA (-).

18/01/2017 | Comenta la paciente que en el centro sociosanitario donde trabaja ha habido un caso de tuberculosis bacilífera en la que tras el estudio de contactos se ha detectado un trabajador y otros residentes. Teniendo en cuenta los antecedentes personales de la paciente y la medicación actual sería recomendable descartar contacto laboral. En el Servicio de Neumología ha sido estudiada antes del inicio del tratamiento biológico siendo negativa para infección Tuberculosa Latente | ||||||

– RX DE TORAX: no condensaciones ni signos de IC.
